Your Patients Group, working in collaboration with
Dartmouth Medical Practice, is undertaking a survey to
find out patients' views on local health care.
To serve patients better, we would like to get your
comments on these services and your suggestions on
ways they might be improved. This information would
help the practice to be more responsive to your needs.
This is also a good opportunity to let us know your
views on the proposed closure of Dartmouth hospital
and Minor Injuries Unit.
The survery should not take you more than 10-15
minutes to complete. It is very important to get a
good response so that the results are as valid as
possible for the whole patient population.
By far the most useful information comes from the
comments and suggestions you may make, so please take
the time to set these down where requested.
All information provided in this survey will be
treated as entirely confidential. No personal
information is requested and any information given
will not be shared with anyone. The survey ends on
June 5th 2016.
Please take this survey as soon as you can to help us
help the doctors and the NHS to care for you!
Minor Injury Unit (MIU) Closure

not be cancelled. It is our intention to work with the
hospital in delivering solutions to ‘plug the gap’
created by the closure, but this cannot include
delivering a MIU within the practice.
Providing safe healthcare in the community is under
pressure and Dartmouth is not immune to problems of
recruitment and retention of staff. Accepting the need
for flexibility in the delivery and receipt of
healthcare will be necessary to overcome the current
Further details will be available in the near future.
