Registration letters from NHS England

The NHS is carrying out work in Devon, Cornwall and the Isles of Scilly to make sure the patient details on GP lists are accurate. Letters are being sent to some patients but not all.

if you do receive a letter, please either confirm your details or submit any changes. Changes can be made by returning the form in the prepaid envelope. If your details are correct you can confirm by freepost, freephone, via text message or online.

It is important you respond to this letter, as GP lists do get out of date over time. If we do not have accurate and up to date information on you then we may not be able to contact you with, for example, test results, invitations to national programmes (vaccinations, health programmes) or to let you know of any changes to appointments or treatments we may currently be offering you.

Please respond to this letter within four weeks so that the NHS can be certain about your details. Otherwise, the NHS will assume that you have moved away and begin the process to remove you from our practice list.

We are happy to introduce our website.

We think we have thought of everything! If not.... let us know so we can offer you the best possible care. We would be grateful if while you are here on the website you could visit our Patient Toolbox and update your personal and medical details. This will help us provide the best possible healthcare to all our of patients.
