Getting forms
completed is easy. All you have to do is bring your form in
to the surgery and leave it with the receptionists stating
which part of the form you want filling in and signing by
the doctors. Please bear in mind that this service is not
part of the NHS, therefore a fee is payable depending upon
the type of form you want filling in. Please see the table
of Service Fee's for more information.
The most common of forms we fill in are:
- Private
Sick Notes
- Driving Licence Forms
- Insurance Reports
- HGV Licence Forms

The surgery has
over 8500 registered patients. However, come summertime, as well as looking
after our patients, we see over 2000 patients here on holiday in South
Devon.
Not only that,
but we also issue over 2000 prescriptions each and every month. That's
11,000 items! Please do bear with us if your prescription goes missing, we
will always do our very best to rectify the situation. |
