
Online Appointments - Registration
To use Online Appointments, you are required to register
your interest. This registration process take place in 3
stages:
Stage 1: Patients register their interest
by filling in the form below.
Stage 2: We take the
necessary steps to register you with "My Vision Online"
which is the software which allows you as a patient to
book, cancel and view appointments. This registration
process generates a unique security token and ID number in
a letter.
Stage 3: Once you have received your
letter, you undertake the final registration steps
detailed. You will then be free to login to the system and
book, view or cancel appointments. |

To register, please fill out the following form. If you
wish to register your whole family, you will need to fill
out the form for each individual family member.

Each family member will require a unique e-mail
address if they wish to use the system.
